I hereby apply for membership in Metro State Faculty Federation / AFT Colorado / AFT which includes membership rights, benefits, and services provided by the organization. I also authorize the Metropolitan State University of Denver Faculty Federation / AFT Colorado / AFT to act on my behalf in fulfilling its organizational mission as duly established through the democratic processes of the union. By submitting my information, I agree to be contacted by a member of MSFF who will provide a membership application (the “blue card.” I understand that when the MSFF treasurer receives my signed application with payroll deduction authorization or dues payment, my membership rights, benefits and services will begin.

Authorization for Membership Dues Withholding for MSFF/AFT Colorado

(Full-Time Faculty Only)

I hereby authorize Metropolitan State University of Denver to withhold $497.94 in 12 monthly installments and to pay in 12 monthly installments, my dues to the MSFF/AFT Colorado/AFT. This authorization may be revoked at any time by me upon giving 60 days notice, in writing, to the MSFF and the MSU Denver payroll office. In the event that members of the MSFF/AFT Colorado/AFT authorize a change in dues, I hereby authorize such amount to be deducted from my pay.
