I hereby apply for membership in Metro State Faculty Federation / AFT Colorado / AFT which includes membership rights, benefits, and services provided by the organization. With this application, I also authorize the Metropolitan State University of Denver Faculty Federation / AFT Colorado / AFT to act on my behalf in fulfilling its organizational mission as duly established through the democratic processes of the union.

Authorization for Membership Dues Withholding for MSFF/AFT Colorado

(Full-Time Faculty Only)

I hereby authorize Metropolitan State University of Denver to withhold $497.94 in 10 monthly installments and to pay in 10 monthly installments, my dues to the MSFF/AFT Colorado/AFT. This authorization may be revoked at any time by me upon giving 60 days notice, in writing, to the MSFF and the MSU Denver payroll office. In the event that members of the MSFF/AFT Colorado/AFT authorize a change in dues, I hereby authorize such amount to be deducted from my pay.
